By reporting adverse events related to our products, you help us ensure both product and patient safety. The information you provide enables the company to fulfill its obligation to report to regulatory authorities. Regulatory authorities require adverse event information related to the product, even if the relationship between the product and its effects has not yet been clearly established.

Medication Name:
Specify when the patient started the medication, including the dose and frequency (e.g., 60 mg, three times a day). Any actions taken regarding the medication (e.g., dose reduction, discontinuation) and the impact these actions had on the adverse reaction.
Description of the Adverse Reaction:
Please describe any changes in the symptoms over time. Please specify any treatments administered for the adverse reaction. Provide the name and contact information of the healthcare provider who administered the treatment.
